Assessment Task 3 Case Studies

Mr Lee is 78 years of age who lives in Heathwood, a southern suburb in South East
Queensland near Brisbane. He lives in a 4-bedroom house with his wife Mrs Lee who is 74
years of age. Mr Lee walks around with a walking aid; walking stick. He has been pretty good
with his mobility and can get around easily using his walking aid. He also wears prescription
glasses. He suffers from hypertension and has been on antihypertensive which helps
manage his hypertension well. His GP has also prescribed him vitamin D as his last health
check-up revealed low levels of vitamin D which put him at risk for osteoporosis. He has
been having issues with his skin as it is dry and very thin. He has been advised by his GP to
put non-perfumed moisturiser to ensure skin integrity but has been struggling to do that as
he would forget and not be consistent with the skin care regime. He likes to go out for walks
with his wife after lunch wearing his favourite sneakers which has seen better days by the
looks of it’s worn out soles. He walks around the block to their local park. He likes to walk
around the park and sit on the park bench and watch the birds. Recently the council has
started work on the park with access to park being closed temporarily. He has two children;
one son and a daughter. His son lives in the US with his family and his daughter lives in
Adelaide with her family. His daughter visits them at least once a year with her children
whom he misses a lot. Mr Lee enjoyed game of Mahjong with his friends at the Chinese
Community centre at Sunnybank on a weekly basis when he used to drive, but due to ageing
his GP has advised him not to drive and since then he has given up driving. His wife does not
drive; thus, both rely on public transport to do their shopping and visit their GP regularly.
Due to decreased mobility he has been putting off his optometrist appointment for 2 years
and cannot remember when he last saw his optometrist. This year he missed the Chinese
New Year celebration at the Chinese community centre which he used to attend every year
and catch up with his friends and relatives. Mr Lee finds it difficult to explain his issues to his
GP as he speaks Cantonese and his GP although of Chinese descent speaks only Mandarin.
Mr Lee enjoys his oolong tea very much and drinks that regularly, but it is a challenge to get
him to drink water than his tea. On a bright Thursday morning Mr Lee was doing his regular
walk with his wife around his block whereby he felt tired thus sat down on a bench at a bus
stop. When he tried to get up, he lost his balance and ended up falling on the ground on his
knee. His wife assisted him to get up and one good samaritan who was passing by helped
them. Upon reaching home, he noted there was a graze on his knee and a skin tear on the
shin of his right leg. Luckily it was the day the community nurse (in this case you!) was
visiting him. This was the first time Mr Lee had any issue with his mobility resulting into a
fall. Given the history of low levels of Vit D and at risk of osteoporosis you are worried about
his fall today and would like to conduct an assessment. You are relieved that you have a
student nurse with you today who has been buddied up with you who happens to speak
fluent Cantonese as she is from Hong Kong. You decide to take this opportunity to conduct
an assessment on Mr Lee regarding his risk for fall.

Description
The aim of this task is to help you in prepare for your WIL experience by applying your learning to
the assessment and care planning for older people in diverse health settings.
Assessment Criteria
The case study responses will be assessed according to the following criteria:
• Understanding of biophysical and psycho-social processes associated with ageing
• Interpretation and use evidence to plan care
• Appraisal of principles of equity, rights and access
• Information literacy skills: use of sources and citations
• Written presentation including structure, grammar, application of Harvard referencing
protocol
Case study responses
You will be provided with 1 case study. For the case study, you will:
o Discuss the biophysical and psychosocial processes pertinent to the case
o Identify an assessment tool appropriate for use in the case
o Discuss 3 priorities of care for the person described in the case
o Address any equity, rights and access issues relevant to the case
Case Study Instructions
• You are required to write your case study responses in your own words. Do not use direct
quotes.
• The template is set at 1 ½ line spacing and Calibri font size 12. Do not change the
formatting.
• Each case study response should be approximately 200 words
Supporting literature to answer the Case studies:
You are required to use Harvard Referencing style as per the guide provided by USC.
For each response, provide in text citations (references) that demonstrate you have read and
referred the most appropriate information to support your response e.g. journal article, textbook,
policy documents etc. You are expected to use a variety of current, credible sources in each case
study response.
Reference List:
At the end of each response you need to provide the full reference details, in alphabetical order, of
each citation (reference) you have used in your case study responses. You are required to use the
Harvard Referencing style.
The completed case study assignment will contribute to 40% of your overall grade mark and is
due on Friday 16/10/20 4 pm
